Interscholastic Sports Physicals
All students interested in participating in an interscholastic sport, must have a physical and health history form.
Dear Parents/Guardian of Athletes,
As advised by BOCES and our school physician, Dr. Peter Kowalski, sports Health History Forms (the check list) for fall sports will not be accepted before August 1st prior to the start of each new school year. This change is for the safety of our school athletes so that we can assure that they are in good physical health in order to participate in the school athletics programs. The Health History Forms that have already been turned in during June of 2019 will have to be reviewed and re-signed by parents/guardians if there are no changes at an upcoming meeting designed for this purpose. The information regarding this meeting is forth coming. Please check the school district web site and the school athletic web site for further information.
In addition, Health History Forms for each sports season will be accepted no more than thirty (30) days prior to the start of each new season. We are sorry for any inconvenience caused by this very important change. Please remember that the safety of our school athletes is our first priority. It is imperative that the health history form required to play a sport is completed and signed by a parent. Children have made mistakes in answering the health questions which delay their clearance to try out or play.
